Records Disclosure
I am a Former Child/Youth In Care requesting information from my file such as:
- A complete copy of my involvement/history
- A copy of any/all identification on file
- A copy of any assessments/medical records on file
- Any personal belongings that may be kept as part of my file (photos, drawings, report cards, letters, artwork, etc.)

Completed forms can be dropped at our head office located at 433 McIntyre Street West in North Bay or sent by email to PrivacyServices@parnipcas.org. Please include a copy of your identity with your request.
If you are sending us personal information by email, please be aware that electronic communication is not always secure and can be vulnerable to interception.
I am a Parent/Guardian and I am requesting information about my involvement such as:
- A copy of my closing letter/safety plan/service plan
- A copy of my record as a resource for the Society (foster parent, kinship service, volunteer, etc.)
- A copy of my file

- Pre-sentence Report
- Canada Revenue matter
- Subsidy Application
- Indigenous Status

Once the form has been completed, pay your $35 fee via PayPal, Cash or Cheque.
